- a completed registration form
- a deposit of $150 USD toward the workshop fee
- a CV or resumé; and
- a letter of intent outlining your interest in the workshop and any specific goals you have for yourself

- balance of workshop fee: JULY 15, 2013
- Please remit all payments in USD checks or money orders. We can accept Paypal but need to add an additional fee to cover the service cost. Please email or call for specific details on making a Paypal payment.

Deposit is non-refundable except in the event that the workshop is cancelled. In the event of under-enrollment, the workshop may be cancelled.
